As you know by now the Biden Administration will hire an army of people to come to your door to push vaccination unto you and your family. If you don't want someone pounding on your door, pushing a vaccine, you're going to need to opt out of the state USIIS database now. These steps are for Utah.
(Every state has its own database and its own opt out. Only Texas is an opt-in state. So for everyone but Texans, you need to opt out. Moving doesn't matter either as it can follow you from state to state.)

1. Request records. You may want to do this if you don't already have records of any vaccines given. You may want proof of it one day.
2. After clicking "learn more", scroll down that page and click "withdrawal form".
